A healthcare facility in Fort Edward, NY is seeking a Director of Social Work (DSW) or Director of Social Services.
\nWe’re looking for an experienced professional to lead the Social Services department. This role includes staff supervision, care planning, resident support, and coordination of discharge planning. Occasional community outreach may be part of the responsibilities.
\nResponsibilities:
\nOversee the Social Services department
\nSupervise and support department staff
\nGuide assessments and individualized care plans
\nAssist residents and families with adjustment and discharge planning
\nRepresent the facility in community outreach efforts as needed
\nApply sound judgment and creativity to achieve departmental goals
\nRequirements:
\n2+ years of management experience in a long-term care or healthcare setting
\nMSW and LSW required
\nStrong understanding of state and federal regulations
\nExcellent leadership and interpersonal skills
