We are seeking an EXCELLENT Home Care Giver to become a part of our team! You will provide routine care to clients at the client's home and/or in the community.

Responsibilities:

• Provide routine individualized care to individuals across the lifespan including pediatric, youth, adult, elderly, convalescents, or persons with disabilities.
• Monitor and report changes in patient health status
• Assist with personal care, medication, and other tasks as needed
• Perform Housekeeping tasks such as cooking, light cleaning, organizing
• Transport to appointments & activities

Qualifications:

• Experience in hospice, home health, or other related fields preferred
• Positive, patient, and caring demeanor
• Ability to build rapport with patients
• Ability to handle physical workload
• Excellent written and verbal communication skills
• Works well independently
