Your Role as a Home Health PTA

Here’s a snapshot of what you’ll be doing:

• Engage in ongoing interdisciplinary assessments of patients.
• Follow the treatment plan set by the physical therapist.
• Collaborate with patients and caregivers to develop and evaluate care plans.
• Deliver services as prescribed by the physician.
• Assist in documenting patient progress and treatment responses.
• Ensure timely and accurate documentation of all treatments and interventions.
• Educate patients and families to support their healthcare needs.
• Communicate effectively with the healthcare team to coordinate care.
• Access necessary personal health information to fulfill your duties.

What We’re Looking For

To thrive in this role, you’ll need:

• Current licensure as a physical therapist assistant in Michigan.
• At least two years of experience as a PTA within the last five years.
• Active CPR certification.
• Previous home health experience is a plus!
