Shift: Days | Schedule: Monday–Friday, 40 hours/week + 1 weekend/month

Assignment Duration: 13 Weeks with possible extension.

Guaranteed Hours: 40/week

Job Description:

We are seeking a dedicated and experienced Home Health Physical Therapist for a full-time contract opportunity in Sacramento, CA. The ideal candidate will provide high-quality care in a home health setting and be comfortable managing documentation, communication, and occasional weekend rotations.

Key Responsibilities:

• Prioritize Start of Care (SOC) visits
• Supervise Physical Therapy Assistants (PTAs)
• Submit SOC reports by the next day
• Complete clinical documentation within 48 hours of visit
• Communicate promptly with the clinical supervisor and office team
• Maintain responsiveness during business hours.

Requirements:

• Active California PT License
• Minimum 1 year of recent home health experience
• Familiarity with the Kinnser (WellSky) documentation system
• Ability to meet a productivity goal of 30 points/week
• Reliable transportation (company car available with reduced bill rate).
